Eligibility Criteria:

• Individuals who are elderly or with a disability but:
• Must not require human assistance for 2-4 ADLs and semi-dependent in ○behavior, medication administration, and mobility; or
• Must be dependent on 5-7 ADLs, most especially mobility
• Individuals who need expert nursing support for their health needs
• Individuals who are at risk for a nursing home placement, following their need for expert support

• For individuals participating in the Money Follows the Person project, environmental modifications, and assistive technology for 12 months following a move to the community from a hospital or nursing home
