We know the stigma that comes with HIV and/or AIDS, and we wish to help individuals suffering from this disease get the support they need under cost-effective means. The HIV Waiver aims to keep individuals at home or any chosen residence without compromising the level of care given to them during their stay.

Eligibility Criteria:

• Individuals must be suffering from the human immunodeficiency virus (HIV) and experienced a decline in their functioning because of the diagnosis, requiring expert support given by nursing homes or facilities
• Individuals must have had an inpatient hospital admission within three (3) months before the request for a waiver
• Individuals must pass the financial eligibility criteria

• Enteral Nutrition
• Transition Services
• Personal Emergency Response System (PERS)
• For individuals participating in the Money Follows the Person project, environmental modifications, and assistive technology for 12 months following a move to the community from a hospital or nursing home
