The MR/ID Waiver is especially concentrated on helping individuals with intellectual disabilities, supporting their home and community integration and an improved quality of life.

Eligibility Criteria:

• Diagnostic: Individuals must secure a psychological evaluation stating their intellectual disability diagnosis and how this affects their daily level of functioning
• Functional: Individuals must be able to pass the ICF-MR (Intermediate Care Facility [for persons with] – Mental Retardation/Intellectual Disability) eligibility criteria through the Level of Functioning Survey done
• Financial: Individuals must be determined by a representative of the Department of Social Services to be financially incapable of availing of their care without the support of the program
